Prior Authorization Forms

To obtain Prior Authorization for a specific drug, your prescribing physician must submit the appropriate form(s) from the list below to the Sterling Retiree Prescription Drug Plan. If necessary, your doctor can call our Customer Care Center (800-313-7667) and request that a form be faxed, e-mailed or mailed to him/her.
